School Age Program - Waitlist Form

Any parent/guardian of a child who may be enrolled in Casa Central School Age Program must meet the following requirements: He/She must be working, in employment/training, or in school for at least 25 hours/week and must meet IDHS household income eligibility guidelines.  Any child who is to be enrolled in the School Age Program must be between the ages of 5-12 years, enrolled in school, and be able to function in a group environment.
